Sorting through information and securing opportunities for a loved one with a disability can be an arduous and challenging process. Project Connect provides trained and understanding staff to expertly guide callers regarding services.
Project Connect staff are available five days a week to respond to inquiries and offer assistance and support. If we are unable to provide answers to inquiries or direct individuals to services within ADAPT Community Network, we will offer referrals to other agencies and organizations.
Care Managers work with individuals and families to ensure access to services, monitor satisfaction and help in the journey to greater independence. Our professional service coordinators take great pride in their ability to form meaningful relationships with the people they serve.
